Provides forms to send to SCERS to change address, beneficiary, taxes, and choose or change direct deposit. Also provides information regarding dissolution of marriage.
Suspension of Retirement Allowance
Persons who receive a retirement allowance from the Sacramento County Employees’ Retirement System (SCERS) for past employment cannot work for any SCERS employer on an indefinite or “permanent, regular” basis. However, persons who receive a service retirement allowance and are offered permanent, regular employment with the SCERS employer from which they retired may suspend their allowance. Member’s Affidavit
This form is mainly used to change your address, beneficiary, or marital status.
Authorization for Direct Deposit
This form is to sign up or change the direct deposit of your retirement warrant. It is also used when submitting the Service Retirement application.
Note: Banking information changes must be received by the 15th of the month* to process timely pension payments. Any request received on or after the 16th of the month will be processed the following month. Untimely notification or incorrect banking information will result in a payment delay of up to 20 days. Any rejected direct deposit will be issued by check and mailed directly to the address on file.
*Processing deadlines for the months of February and November occur earlier in the month.
Dissolution of Marriage after Retirement
This document provides information and answers questions about the division of retirement benefits upon the divorce or legal separation of a retired member. This information is for members who are already receiving benefit payments from SCERS.
Request for Informational Letter or Document
Use this form to request reprints of documents previously provided by SCERS or to request necessary informational letters. Note: If you are requesting an account balance statement to supply to a third party using a specific date range, please submit the formal third-party request for consideration. Tax Withholding
This form is used to change your Federal and California State income tax withholding amounts.
